Service Description

This formulation is for TWO recipes formulated at the same time. This appointment type includes veterinary history review, a 45-60 minute consultation (online or telephone) and two therapeutic recipe formulations with detailed report including nutritional analysis and monitoring recommendations. Recipes are formulated FEDIAF and current Veterinary Clinical Nutrition guidelines for the relevant condition. Veterinary History: A full veterinary history is required before your consultation. Please ask your vet to send it to: charlotte.gray@companionnutrition.co.uk. A report will be sent to your primary vet after the consult. Common Conditions Treated: Pancreatitis Renal Failure Food Intolerance/Allergy IBD/IBS Acid Reflux Urinary Stones (Urate, Struvite, Oxalate) Cardiology Support Cognitive Support (Elderly Pets) Weight Loss Complex/Combined Conditions About the Service: Consultations and formulations are undertaken by Veterinary Surgeon Dr. Charlotte L. Gray MA (hons) VetMB MRCVS (Cambridge 2009). Reports are typically delivered within 2 working days. Cancellation Policy Email cancellations to: charlotte.gray@companionnutrition.co.uk. Cancellations made more than 7 days from your appointment date will be refunded in full. Shorter notice cancellations *may* be refunded in exceptional circumstances but may not be refunded in full. Insurance As a veterinary service, you may be eligible for insurance reimbursement (UK only). Direct claims aren’t available, but we can complete claim forms free of charge. Follow-Up Support Follow up support and change to the recipes are *not* included with the exception of enquires relating directly to the original report (for example if clarity is required on the report content, or if a recommended supplement is not available to you). Changes to ingredients or clinical changes in progressive condition management may require a follow-up consult. These can be booked via the online booking system as required.
